THIS PAGE IS FOR ALL NEW STUDENT-ATHLETES TO THE DOMINICAN COLLEGE ATHLETIC DEPARTMENT
All incoming freshmen and transfer students must have their Primary Care Physician complete both the Pre-Participation Athletic Physical and the Health Services Physical.
You are also required to have the orthopedic physical your first year of participation in intercollegiate sports. This examination will be completed in the AT Room by our team physicians. You will be notified by your head coach when these examinations are scheduled. This examination is free for you as a student-athlete. If you are unable to attend, you will be responsible for completing on your own and at your own expense. You will not be reimbursed for the cost.
The forms must be returned to the Athletic Training Room on or before the first day of practice. All forms must be completely filled out before any student-athlete will be eligible to participate in intercollegiate sports at Dominican College. You are not eligible for pre-season workouts, practices, scrimmages, tryouts or games without all the forms completed.
All incoming freshman and transfer students OR any student-athlete that entered Dominican College during the spring semester 2014 MUST attend their team's scheduled Orthopedic Examination and comply with the following in order to be eligible for the 2014-2015 seasons:

2. Attach Sickle Cell Solubility Test Results to Athletics Physical Form
3. Complete the Insurance Acknowledgment Form
4. Attach Front and Back Copy of Insurance Card
5. Complete the Sickle Cell Consent Form
6. Complete the Concussion Form
7. Complete the ADHD Medical Exception Form (if applicable)
